These letter samples are provided for parents/guardians wishing to translate Child Nutrition Services meal eligibility letters into a language other than English using an electronic translation program such as Google Translate.

Please use them if needed, to compare to the letter sent after your meal application was processed.  If you have questions about meal eligibility, please contact a Child Nutrition Services staff member at 253 931-4972.

 

SAMPLE - APPROVAL of FREE or REDUCED-PRICE Meal Benefits Based on Application

Notice of Approval

Dear Parent/Guardian:

This letter contains important information about *school meals for the following students:

   Student Name(s)

Based on information provided, your child(ren)’s meal application is APPROVED for FREE or REDUCED-PRICE meals for the 2016-17 school year.

IF Reduced:

   Breakfast is free for all grades
   Lunch price is $.40 for students in grades 4-12 and Pre-K
   Lunch is free for students in K-3

If you do not agree with this decision, you may discuss it informally with Carol Barker by calling 253-931-4972.  You also have the right to request a hearing to appeal this decision.  If your appeal is filed within ten days after the date of this letter, benefits will continue until the appeal is settled. 

You may reapply for free or reduced-price meal benefits at any time during the school year.  If you are not eligible now, but your income goes down, your family size gets larger, or you begin to receive Supplemental Nutrition Assistant Program (SNAP), TANF, or FDPIR benefits, you may submit an application at that time.

Direct Certification

Dear Parent/Guardian:

This letter contains important information about free school meals for the following eligible student(s):

   Student Name(s)

Do NOT fill out a meal application for the listed student(s).  You do not have to return this notice or take any other action for the listed student(s) to receive free school meals.  This eligibility determination is APPROVED for FREE meals, effective (date will be inserted), for the 2016-17 school year.

Other students in your household who do not receive SNAP, TANF, or FDPIR benefits may be extended free meal benefits based on the eligibility of the listed student(s).  Please call the phone number below if this is the case so eligibility can be extended to a student who is not listed above.             

If your child(ren)'s eligibility information was electronically transferred (matched) from the state agency responsible for overseeing one of the programs (Supplemental Nutrition Assistance Program, Temporary Assistance to Needy Families, or Food Distribution Program on Indian Reservations), it has been permitted under Federal Law.  We may share eligibility information with education, health, and nutrition programs to help them evaluate, fund, or determine benefits for their programs; auditors for program reviews; and law enforcement officials to help them look into violations of program rules.

Please contact our department if you have child nutrition questions, need to add another student to this meal eligibility, or do not want your child(ren) to receive free meals at school.

If you do not agree with this decision, you may discuss it informally with Carol Barker by calling 253-931-4972.  You also have the right to request a hearing to appeal this decision.  If your appeal is filed within ten days after the date of this letter, benefits will continue until the appeal is settled.  To request a formal appeal hearing, write or call Cindi Blansfield, 253-931-4972.
